Can We Finally See Biofilm? Alcian Blue Is Getting Attention

Biofilm has been part of the chronic wound conversation for years, but at the bedside we still do not have an easy way to see it. Culture tells us what grows, and F-imaging can help identify bacterial burden, but neither directly shows the ECM that helps define biofilm.

That is where Alcian Blue wound blotting gets interesting. It binds to negatively charged carbohydrate-rich material within the EPS matrix, allowing wound material collected on a membrane to be stained and assessed. It is not bacteria-specific, so blue staining should not automatically be called biofilm. It is better viewed as a way of detecting biofilm-associated extracellular matrix.

The clinical data are promising. In one study of 31 patients, the modified protocol showed 95.2% sensitivity, 60% specificity, and 83.9% overall consistency with microbiological culture. Of 18 positive-staining wounds, 15, or 83.3%, remained unhealed at 1 month. Another study of 53 patients found Alcian Blue grading was associated with 90-day healing outcome, with a Kendall tau of 0.563 and p < 0.001.

Its potential around debridement may be especially useful: assess the wound, debride, then blot again to see whether biofilm-associated matrix remains.
Alcian Blue is not the new gold standard yet, but it may become a very useful piece of the wound care puzzle.
